The address of every place I've lived for the past ten years.  For me, ten years went back to freshmen year in college and included seven different addresses.  There were only like four blanks, but since this was the Official Background Check Form Without Which We Won't Look At Your Application, I wedged all seven in there.  That was the longest application of the 50+ schools I applied to last year.  I got the job, probably because I was the only person to actually fill out the whole application. 

Wow. The federal background check I went through for my current job only went back 7 years, and I thought *that* was a b*tch.

I too thought those forms were the most annoying HR nonsense ever until this week when I received an AA EOE form without any postage-paid way to return it (or online option).  A small thing, I know, but this broke applicant begrudges them my .44 cents and envelope.

High school attended, graduation date, class rank, and average
name of a HS teacher who will provide a recommendation
 
SAT scores
GRE scores



Good heavens.  I'm pretty sure that most of my high school teachers are either deceased or in nursing homes by now.

If married, names and ages of any children
(obviously, if not married, I don't need to tell them about my children).

Today I filled out 8 pages of excrutiating detail in one of those online, can't save it, print it and fax it, sort of forms. Then, the browser crashed and I had to start all over. Ugh.

And, having since retyped everything and printed it, I notice that although it appears to give you a large window for addresses, it only shows the first two lines when printed. Pfft!

Ugh, I am trying to apply for a job now and failing miserably. Thankfully they only require a CV, cover letter, and dissertation summary. Unfortunately, every time I try to upload my PDF documents I get the error "the parameter is not correct." No hint as to what parameter they mean, and I've searched the HR site and made sure all my drop down menus are properly selected. What a pain!

I had to do this twice for my school, as I changed position types.  Nevermind that they must have had my old forms.  It was a b*tch the first time (my brain has done me the favor of blocking out most of the inane info that I was asked to provide) , but, fortunately, I kept a photocopy of it.

I had to fill out one of these long HR forms for what seemed like a great fit at a SLAC out east.  After taking the hour to type out everything on my CV, I received an email from the school.  I just assumed it was a generic confirmation saying that they had received my application.  I wish!  Apparently, the search had been cancelled but no one bothered to remove the job ad or the application from the Human Resources webpage! It's one thing to fill out an app and not make the short-list - but to complete the app after the search has been disbanded was reaallly aggrevating.
