These are particularly common in medical fields, I know, and my field is tangential to a medical field.  A student has just emailed me a draft of a paper that has Student, other people, Drspouse, Corporate Author. Typically we'd have Student, Hoi Polloi 1-5, Supervisor.

I know some journals don't take these types of author (though this paper is intended for a medical journal so we should be fine).  Other than that what would people interpret/suggest for the authorship order? Is it more typical to have CAs middle, or beginning, or end? If end, do people still assume the supervisor is the final (i.e. the CA), or the penultimate (before the CA)?

We also have a dual-supervisor issue but I am going to magnanimously suggest I take half and the other supervisor takes half.

I don't know about this, but I would put CA second (after student) and keep supervisor(s) last.
