Assessment of instructors in online courses

I'm teaching a course online for the first time this summer, and have found the discussion in these boards to be helpful.
I am setting up the eval of me as instructor - I have the standard questions we use in face to face classes already established.
I have the option to add extra questions more tailored to the course and online teaching - what your thoughts about doing so?

Sure, why not? The usual questions aren't terribly helpful -- this will allow you to ask what you want to know, and get into specifics. It'll also allow you to ask about their efforts and what they got out of the course.
There are lots of threads on evals. This one is for midterm evals, but it had some questions and approaches that you may find useful.

I have come across the same problem. One of the issues is the distinction between the course developer and the instructor who delivers the online course. Sometimes students do not understand this. Fortunately, my school has set a special set of questions and criteria for evaliuating the online instruction. So if in your case your online instruction is being treated in the same way as face-to-face instruction, you may want to raise this issue before the director of the office of the technology-based learning or whatever it is called in your school. I am expecting that your department head would appreciate this.
