It's usually a one- or two-page statement explaining your pedagogical philosophy -- what you think constitutes good teaching -- and your teaching objectives, followed by specific examples of what you do in your classes and how your philosophy relates to what you do. Don't simply list the courses you've taught or throw out a lot of buzzwords. Don't underestimate the importance of your statement of teaching philosophy either, especially if you're applying to liberal-arts colleges that place a premium on teaching.
